Our Policies

WELCOME TO OUR PRACTICE!

OUR GOAL IS TO PROVIDE QUALITY CARE TO OUR COMMUNITY IN A SAFE AND CARING ENVIRONMENT.

Appointment

We make every effort to see you on time for your appointment. We ask that you arrive 15 minutes prior to your appointment. If you arrive after your appointment time, you will be asked to reschedule.

Cancellations

We ask that you cancel at least 24 hours prior to your appointment whenever possible. Messages left on our answering machine are checked each morning when the office is open.

Snow Policy

In the event of severely inclement weather, we will make every effort to contact you regarding cancellations. Please confirm with our office prior to your appointment.

Referrals

We are happy to see you at your doctor’s request, and we will send office notes for each visit. If you are self-referred, we will keep your doctor updated unless you request otherwise.

If you require a referral from your insurance company, you will be responsible for contacting your primary care doctor’s office for that referral. If we do not have that information at the time of your visit, you will be asked to sign a waiver prior to your appointment.

Prescription Requests

If you have been provided with samples to try and would like a prescription called to your pharmacy, please allow up to 24 hours to complete your request. Refills can be requested through your pharmacy.

Financial Policy

Payment for services not covered by your insurance is requested at the time of service. This would include any office co-pay as indicated by your insurance company.

Payment is due in advance for surgical procedures not covered by your insurance company. Please ask about our finance options with CareCredit and NA which we only accept for audio purchases.

 

Emergency and After Hours Calls

In the event of a life-threatening emergency, dial 911. Urgent calls made to our office during business hours will be directed to our medical staff. If you wish to leave a message for the next business day, you can call (603) 432-8104, for more immediate attention you will be asked to dial 9 to reach our answering service and the provider on call with be contacted for you.
